Health History Consent (pdf)
The Health History Consent Form is required of every participant in an Outdoor Classroom Program. Health History forms are due to Chewonki the Wednesday before your program.

Group List (pdf)
The group list form should be filled out and turned in to Chewonki with the Health History forms. Be sure to include the teacher chaperone's name at the top of each group and an emergency school contact number.
